About this deal

Active ingredient w/w: Menthol 2.54%, camphor 1.43%, methyl salicylate 0.42%, oleoresin capsicum 0.005%. Contraindicated where there is known hypersensitivity to aspirin, other salicylates, or other non-steroidal anti-inflammatory drugs (including when taken by mouth) especially where associated with a history of asthma. Radian B muscle rub is a cream containing menthol, camphor, methyl salicylate and capsicum oleoresin. It works by reducing inflammation and by causing counter-irritation in the area where it is applied, which helps to soothe pain in the underlying tissues.

Radian B is a non-selective inhibitor of cyclooxygenase, an enzyme invovled in prostaglandin synthesis via the arachidonic acid pathway. Its pharmacological effects are believed to be due to inhibition cylooxygenase-2 (COX-2) which decreases the synthesis of prostaglandins involved in mediating inflammation, pain, fever and swelling. Antipyretic effects may be due to action on the hypothalamus, resulting in an increased peripheral blood flow, vasodilation, and subsequent heat dissipation. Inhibition of COX-1 is thought to cause some of the side effects of ibuprofen including GI ulceration. Radian B is administered as a racemic mixture. The R-enantiomer undergoes extensive interconversion to the S-enantiomerin vivo. The S-enantiomer is believed to be the more pharmacologically active enantiomer. Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. Just because a side effect is stated here, it does not mean that all people using this medicine will experience that or any side effect. Uncommon features include haematemesis, hyperpyrexia, hypoglycaemia, hypokalaemia, thrombocytopaenia, increased INR/PTR, intravascular coagulation, renal failure and non-cardiac pulmonary oedema. Apply the lotion on the affected part once or twice, leaving 10-15 minutes between applications. Smooth in or massage if preferred. If necessary, repeat application up to three times daily, reducing to morning and evening when acute symptoms subside. Frequency not known: Acute generalised exanthematous pustulosis (AGEP), photosensitivity reactions.

Interaction

Keep away from eyes, irritated or broken skin and sensitive areas. Use sparingly on tender skin and do not cover immediately after application. Haemodialysis is the treatment of choice for severe poisoning and should be considered in patients with plasma salicylate concentrations >700mg/L (5.1mmol/L), or lower concentrations associated with severe clinical or metabolic features. Patients under ten years or over 70 have increased risk of salicylate toxicity and may require dialysis at an earlier stage.

The side effects listed above may not include all of the side effects reported by the medicine's manufacturer. Therapy with NSAIDs should be administered cautiously in patients with preexisting fluid retention, hypertension, or a history of heart failure. Most patients who have ingested or been exposed to clinically important amounts of NSAIDs will develop no more than nausea, vomiting, epigastric pain, or more rarely diarrhoea. Tinnitus, headache and gastrointestinal bleeding are also possible. In more serious poisoning, toxicity is seen in the central nervous system, manifesting as drowsiness, occasionally excitation and disorientation or coma. Occasionally patients develop convulsions. In serious poisoning metabolic acidosis may occur and the prothrombin time/INR may be prolonged, probably due to interference with the actions of circulating clotting factors. Acute renal failure and liver damage may occur. Exacerbation of asthma is possible in asthmatics. Patients who have previously shown hypersensitivity reactions (e.g. asthma, rhinitis, angioedema, or urticaria) in response to aspirin or other NSAIDs. The mechanism may be due to a combined local effect as well as inhibition of prostaglandins leading to decreased integrity of the GI lining.You can apply the gel in this way up to a maximum of four times a day. Leave at least four hours between applications. It's best to protect the areas of skin you have treated from bright sunlight, because the gel might make your skin more sensitive to sunlight.

In children aged four years or less, a dominant metabolic acidosis with low arterial pH (raised hydrogen ion concentration) is common. Acidosis may increase salicylate transfer across the blood brain barrier. Acute Overdose

Squeeze 5 to 10cm of the gel (containing 50 to 100mg of ibuprofen) from the tube and lightly rub into the affected area until absorbed. For adults and children over six years of age, the cream should be applied to the painful area and slowly massaged in. For muscle strains and stiffness it is best applied after a warm bath. Flammable: Do not use near naked flame. Should not be placed on or used near polished or painted surfaces.
